Questionnaire or Testimonial Template request please

After a piffling 12 years, I think its about time I offered a Questionnaire sheet, so my beloved customers can say what they really think of me.

I've checked the forum but couldn't find anything suitable.

Could somebody point me in the direction of such a beast, either online or the ol. papyrus type.

I think you use PCRT? Just use the Thank You letter with a link to your testimonial form

I stopped doing the letters as I was getting too many. You'll be surprised the number of people who look for a tech then pick me based purely on testimonials. They really do work

Thanks, that is the big issue I currently have, because people can view my website etc but have no idea who I am. I am designing a new WordPress website so that will become apart of it.

Why not use a service such as the Freeindex? I think the reviews have a little bit more creditability when they are not listed "directly" on your website.

Last Autumn and Spring after we completed any residential repairs/support we asked that they complete a short survey using SurveyMonkey, we had 100% take up.

Also SurveyMonkey is FREE

I sent this out yesterday evening via email. This is more of a feedback request than a testimonial request. The idea is that I'll ask the responders if I can use their feedback as a testimonial. Not sure what sort of results to expect, but if anyone is interested I can post an update after I (hopefully) receive responses. Disclaimer: I didn't write this, I compiled it from several different templates I found and tweaked it to work for me.
