Looking for OCR software that can index PDF?

A client of mine has started a mission to basically document everything she ever done. This is done by scanning things like old insurance papers, kids birth certificates etc. I am not looking for any OCR-software that converts it to text, I would rather find something that can index everything like it's scanned but I will be able to search for keywords, phrases etc.

I know Evernote has this function but I do not think it is sufficient for this purpose, I need something that can make an indexed library of scanned documents. Any ideas?
 
I have a Fujitsu Scanner and the included ScanSnap software is awesome for this. As fast as you can scan in documents, it's OCRs the text and saves them all as searchable PDFs. Does it all on the fly. Not sure how it does at processing PDFs that are already scanned, haven't really tried that. I know it also has the Rack-2-Filer software which can be used to process OCR and index the text of lots of PDFs.
 
The "automatically convert" option doesn't seem to have much effect, uploaded documents 30 minutes ago but nothing has been converted
