Hi, I have an old Pavilion ZX5060us notebook that is doing the BSOD now, ever since I created a UBCD4WIN bootable usb flash drive with it. I ran antivirus scan and reinstalled XP, drivers all look ok. Then I ran a diagnostics cd that told me the keyboard controller on the mobo failed. Does this mean I have to replace the whole board? I'm not afraid to work on this thing, I've replaced just about everything else!

try running a bootable live Ubuntu cd. you can download it.
If the laptop functions normally. then i would just do a clean install of XP from an OEM disc.
If the system is still not functioning normally, then i would first test the
- keyboard
- harddrive
- motherboard
and replace what fails.
You may want to consider the amount of money you already have spent and are about to spend and consider a new laptop if you have already replaced everything else.

I've done that, used some of its tools, memtest, hd tests, etc. couldn't find a prob, so I did completely format and reinstall xp, still blue screening.

Are you using something like Eurosoft PC Check, by any chance? Most laptops do fail the keyboard controller when they're actually nothing wrong.
As for why it's still blue screening, I'm unsure, but I'd guess that it's unrelated to the keyboard controller.

It came from a shop, It has all the system tests combined with scripts. One usb port has died too, you have any ideas what I can try? I haven't spilled anything on it.

there is not much you can do with a laptop since its harder to perform a process of elimination by replacing parts and testing - due to near total integration - like do can do with a desktop.
Since, as you said, you have already tried Ubuntu live and replaced most replaceable or serviceable parts, i'd consider cutting your losses and use the machine for parts.
If you haven't replaced the MB yet and you can find a reasonably priced one, try that but beyond that - i would junk it.
I was given a older laptop a while ago with a password protected bios and harddrive. I just purchased and replaced the MB and used a extra harddrive i had in my parts box. I use it as a print server.
