Outlook Notifications not appearing

I have a client whos Outlook Notifications aren't popping up. Normally they set a meeting on their calendar with about a 15 minute reminder. What they're normally used to seeing, even (especially) if the window isn't in focus is something like this annoying them...

They want that, to remind them of their meeting. But for whatever reason, that screen has decided to stop popping up in Outlook (2016). Here's what we've tried to try to jumpstart this again...

  • Fully update Windows + Outlook
  • File - Options - Advanced - Show Reminders is ON (Also tried turning it off then back on)
  • Recreated under a new Outlook Profile
I've sat there and created an appointment and watched it tick by with no notification popping up like in the image. But after the new profile, we tested and it worked no problem. Now, client emails me saying "Well that worked for a little bit..." I'm not entirely convinced something else isn't going on, like maybe Windows 10 grabs it and puts it somewhere else. At this point I just don't know, but anyone with insight, I would appreciate it. They're the kind of person who relies on these to remember meeting times.

Did notifications somehow get toggled off for Outlook in Settings, System, Notifications & Actions?

If Outlook is installed there should be an individual entry for it that either allows (if on) or disallows it showing banners and producing sounds.
 
Sorry for the delay, she wasn't in rest of the week. But that said, I checked and Outlook was indeed "ON" in the notification center. I did 3 separate tests over the week at different times and each time it did come up. All I had was Outlook opened and minimized in the background as something like Chrome was up.

So now, even though I've seen the notification just not pop up once before with my own eyes, I'm starting to think maybe it's user error. Maybe she clicks away quickly or there's some action the user can take that will cause it not to trigger. I left the 3 events "up" for her to see and sent an email to let me know if this continues to happen, so we'll see...
