Remote Assistance-Work order?

Forgive me if this question has already been answered. I looked within this forum, but didn't find what I'm looking for.

What do you all use, if anything, in the form of a work order when providing remote assistance? The paper work order I have covers business terms like I'm not responsible for lost data, etc. etc. I'm trying to come up with a way to get this to client's when providing remote support.
 
Another option is to use a fillable form on your computer and then save the file as PDF. Place a copy on the clients desktop and you can also print it out for them.

This is what we do and it works well. We also use a web form that allows the client to enter initial information but most ofm the type the customer is not very specific about the problems.
 
Yup, that's what I did. After they fill the form the page redirects them to the download page for the remote software. As soon as the form hits my inbox and I verify it we start.

About a year ago I spent a few days constructing a PDF form that could be filled out online. I gave that form up for MHelpDesk and I now spend more time helping my customers than I do with administrative tasks... the techs you reach at MHelpDesk are nothing but awesome, and are always willing to customize your forms for you.
