[SOLVED] Never seen anything like this

What wireless card is in it and what chip is it using (Atheros, Broadcom, etc)?

What model of laptop are we talking about here?

Is it using Windows Zero Config or it's own wireless manager? Perhaps try deleting that profile for whatever manager is being used and create a new connection.
 
I got around to solving the issue. The wireless card was no good. It turned out that it wasn't just the router, it did the same with several routers. Just not the particulars ones in my office or some other places it worked. After changing the wireless card it was fine. The original wifi card was an intel.
