We have tried several strategies to handle this situation including charging the same for virus removal and new installations. Then only difference then is whether they need a backup as well.
It always seems there is that customer who has more trouble than the virus and we sometimes do not know of it till they get home and now there are angry at us and they have to do the process twice even if for free we wasted their time. For this reason we push towards a reload whenever we see a heavily infected system and we always warn of this possibility when the customer insists on a virus removal over a installation.
Fixing Windows since the beginning that is just something that Microsoft has always pushed us to do back in the Win95 days through 2000+
There is even a word made up for it Windows code rot. I hate to work on a system with an old installation more than 1 year old.
