We ended up deleting and recreating the account on the iPhone and this is all working fine now.
However whilst waiting for me to come back with some solutions, he changed his Google account password, which killed the Outlook setup on the computer.
After much unsuccessful fiddling with Outlook, I deleted and recreated the account here as well.
Receiving seems to work fine but sending is impractically slow to the point of having to restart Outlook multiple times just for a test message to go though "Send immediately when connected" is selected.
I had created an alternate profile to test, same thing there as well.
Have repaired the Office installation.
What am I missing?

If not, you should.

If they're really insistent about using Outlook on their desktop I'd recommend they switch to Microsoft 365. Google Workspace is for the browser. Microsoft 365 goes with Outlook.

M365 desktop apps are the defacto default desktop apps for GSuite. Google claims these things aren't required, the market... quite another.

Microsoft does service and support this market.
