anyone got an "account agreement" form they would care to share?

I have struck up a partnership with a local mobile phone retailer and am doing their laptop repairs (insurance jobs). They have approached me asking to open a monthly account (up till now they have been on the phone with a credit card payment each time which is time consuming for both parties).

Anyone care to share the paperwork they use for new account customers?
