PC Doctor Report

I'm thinking about making a report sheet for computer health checks that I can give to customers, in the past I've seen a couple of people mention PC Doctor's diagnostics software produces nice reports but I've been unable to find any pictures of one online. If anyone has the chance, would you mind uploading a report?

I haven't purchased PC Doctor but I have a link to the simulation http://www.pcdservicecenter.com/files/simulation/stream/reports.htm if you follow all the prompts you eventually get to scroll through an example report. Hope that helps!
