My Multi-Part Workorder Form (member requested)

Well I'm getting the 404 error message, too.

Here's a few of mine if anyone's interested.

Work Order

Work Order is used on literally any service or product sold, business or residential, it gets filled out. The top portion gets filled out before anything, I put my name as the service provider and fill out the first line of the itemized statement so the client knows what they're paying, then I have them sign. After everything's done, finish the line items, give any recommendations, fill out the payment stuff and pull the yellow copy for the client. All notes about what happened go either at the bottom or on the support ticket that is available for the client to view 24/7 online. Work Order duals as the receipt as well.

Repair Order

Repair Order is used whenever I take possession of a computer system or hardware.

Forensic Services Request

Request is used whenever a law firm or agency wants to give me lots-o-money. lol :D

Edit! Forgot to mention, I had full-page NCR forms made for me. Filled out their online order form, uploaded my PDF file and they sent me 250 for about $43. Highly recommend them! http://www.anchorside.com/carbonless.html

@NightKingdoms: I've never seen a work order form contain verbiage about possession of a deadly weapon! :eek:

"... I authorize NK to carry deadly weapons on the premises under my control; ..."

I was wondering when someone was going to pick up on that. lol

1) My day job is with the sheriff's department
2) I live 40 miles from the Mexican border
3) Someone has tried to rob me with a knife before

Where I do computer repair guns are really not uncommon to have an carry. We're one of three main thoroughfares of drug trafficking in the state. Most of the people that live here are sheriff's deputies, Customs, Border Patrol, Immigration and Customs Enforcement (ICE), DPS, etc.

About the one time someone tried to rob me, it wasn't even a client, it was as I was leaving. I have heard of one tech in town get a fake service call and the guy had a bat, thinking if he charged so much, he must have a lot of money.

But really there isn't much crime, just the potential for it like anywhere else. I would be miffed if someone robbed me, but for those criminals that don't like witnesses or the little money they get from it, my life is not worth their greed and stupidity.

(Should also mention that I also do CCW classes for people to get permits. lol)

Thanks for Sharing! I'm just starting up and am wanting to get all this kind of stuff together before I launch, really helps to be able to see what other's are using
 
The NCR forms were made by
Anchor Side Printing. All I do is just fill out their form, choose my options (e.g. yellow/pink copies, bound/loose, etc.), upload the PDF and they send the them. Pretty cheap, too.


I am a fool, I misread the NCR part. I read it as NDA <- Non disclosure agreement. I was thinking you had one of those going.

Sorry about the dead link! I completely redesigned my website and also found a new web host so it was not intentional :) I'm extremely busy for the next couple days, but I'll PM those who PM'ed me and let you know when the link is live again :D
