How do you document problems/fixes?

Yes, but it's been a while and I don't remember which programs they were. Did some Google searches and found ConnectedText, plus found some reviews of CT and the reviews often compared it to other programs, which I then went to and checked them out.

Software

I have seen the PCRT. Is there any suggestions, templates, or free software for asset tag data management for the computers?
 
I'm using vTiger for my repair and CRM system. It has an FAQ system which I use for fixed. I put the subject in the Question (e.g. Steps to add computer to SBS 2008 Domain) and document the method in the Answer area. It's text searchable.
 
I take detained notes of the problems, attempted fixes and successful fixes when I work on a system, and enter them into the invoice when I prepare it with QuickBooks.

This is similar to what I do but I don't use QB. I often consult Technibble through the search feature to see if any of the posters have dealt with the issue before. I find that helpful because usually someone there has encountered the same issue before. If that doesn't work, I do web searches as well and bookmark them for future reference or place them in a "solutions" or "tools" folder on my work desktop.
 
In addition to my notes in QB, I use EssentialPIM Portable (notes section only) to save tidbits gleaned from here and elsewhere on the web. I like that it's portable and saves links and images by just copy/pasting the web page or selection thereof. It's searchable, too. I keep it in my "Favorite Programs" folder, which I copy to a 16GB flash drive regularly and always have in my pocket, so those notes are always handy when i need them on-site.

Edit: I lied; it doesn't save links just by copying them. Been trying too many programs like this and must have been thinking of Evernotes, perhaps? It does copy images though, so it's cool for copying articles from web pages.
