Generating a PDF invoice based on input on PDF form

Anyone know of any possible way to do this?

I have a PDF form that I created in Word. I imported it and created form fields in Acrobat Pro XI (the newest one). Now I need it so that when I send the form to a customer, they fill out the form and either hit a button when done or just have it automatically create an invoice using the information the customer put into the form. Is this possible? Any advice/help would be greatly appreciated.

Have it on your website as a fillable form and let the submit button send it to you??
