I just heard back from this customer this morning, a new Verizon router has solved the problem. I'm filing this one under new hardware vs. old hardware.

Been following this thread. I gotta say, looking back over time, I've learned just not to be fond of telco provided gateways. Going back to the earlier days of snet/sbc, with those 2Wire gateways they used for residential installs. (a bit after those Effecient Speedstream 5260 modems) UGH I hated those 2 Wires. Weird things that send you down rabbit holes for weeks before your see the light of day again.
