I wouldn't throw out the monitoring only plan, I would just price things differently. I have a three level (4 if you include break/fix) plan for businesses. Monitoring only w/ antivirus, Proactive Maintenance, and Everything included (minus hardware). The All Included plan I wouldn't do unless they sign up for every machine/server as it can get to hairy if businesses pick and choose which ones they want covered. But the Monitor Only or Maintenance Plan I could let them choose to just have a server.
The idea behind having a monitor only or maintenance plan is to provide a customer with a "sticky" product, which makes them more likely to choose you for service over a competitor. You don't know if your competitors are having a Managed Service type talk with your customers now, so you should have one first. Adding value to the Monitor Only plan by including antivirus helps with the sale. They most likely will already be paying for antivirus so why not get monitoring too?
