Hey all,

I have spent a few mins trying to find a simple script or code to make a easy side feed back button that pops up a small form that clients or new customers can use to say what they would like to see on your site or services.

Look up jquery slide forms in google.

You have not specified what type of site you require this for. But I'm sure a google search will find you what you need.
There are also quite a few CSS ways if you take a look as well
