I have a client who has machines that are in use 24/7 for access to a medical system over the internet.
It is difficult to find a time when the machines are not in use.
Is there a remote solution that would allow remote access that wouldn't interrupt the end user and they could continue to work?

Terminal services or citrix on windows server.
I don't know of any non-unix type systems that allow a second session on a workstation.
I think there were some hacks at one time that allowed it on XP.
EDIT: Apparently you could do it on XP once upon a time but the feature was removed. there are hax available.

The machines are XP Pro. Anyone have any links to the hacks?

that might be against the rule to link to those.

Yeah, and break several EULA's whilst your at it.
I would've called it a work-around, instead of hax :p
Kaseya IT toolkit will give you access to the command line while someone is logged on if that helps
